	Please fill out the below form and click the submit button when done and you will be registered for the New Student Orientation on 21 August 2009 at 10:00 AM in Memorial Hall. Please remember this is not a Mandatory meeting, but it is highly encouraged that you attend to complete the required paperwork due to the large amount of new students enrolled in the AFROTC program this year. Doe) UNCC ID Number: (i.e. 800111896) (this is to make sure we have the right person) E-Mail Address:* (please use your UNCC or Home School e-mail address) Contact Number: (i.e. 704-555-1234) Will you be attending the NSO?: (Yes or No) If not attending the NSO please state a reason: (i.e. conflict with another appointment, etc.) Will your parent (s) be attending the NSO?: (Yes or No) If Parents attending how many?: (i.e. 1 or 2) After submitting the above form you should receive a confirmation e-mail back to your UNCC e-mail address stating that you have been added to the list of those attending the NSO or that we received your notice that you will not be attending.
